Instructions for NASA C-22 form

The C-22 form must be accompanied by a completed NF1676 (Document Availability Authorization). Send the completed forms and the report to the Publishing Services Coordination Office (PSCO).

1. Authors
Persons who performed the work described in the Scientific and Technical Information (STI) report, who can effectively defend the main technical content to a peer group (Unless someone else is designated as a contact person, questions about the report will be directed to the first author, who will be contacted to check the report.)

Affiliation
To be chosen from the pull-down menu

Org. code
The author's assigned organization code

Phone number
Onsite number at which the author can be reached during business hours

Mail stop
Location where mail will be sent

Room number
Room to which printed copies will be delivered (first author or contact person only)

Contact person
Person designated as contact instead of the first author

Report title
Must match title on NF1676 (The title is needed to ensure that documentation stays with the correct job.)

2. Funding division number
Org. code of division that paid for the research upon which this STI is based

Current funded WBS or Cost Center number
Number in this format xxxxxx.xx..xx.xx representing the valid, funded, current fiscal year task number

Subject category number
Used to catalog NASA STI reports and to determine recipients for Standard Distribution reports (Choose up to three numbers from NASA's Scope and Subject Category Guide.)

Subject terms
Used to index and retrieve information in the NASA scientific and technical information system (Must be from NASA Thesaurus. Choose up to seven terms.)

Are trade names used? _ Yes _ No
Mark a box to indicate whether any trade names (e.g., Tyvek, Cray, Teflon, IBM, Cessna) are used in a report so that the proper disclaimer can be displayed on the back of the title page.

3. Report type
(See NPR 2200.2, section 2.3)

NASA published, given a NASA report number
Classifications of material published by the NASA STI program (e.g., TM, TP, CR, and so on)

TM (Technical Memorandum)
Preliminary findings or findings of interest to a limited number of readers with no extensive analysis

TM and Presentation
NASA-numbered report version of findings presented orally at a conference, symposium, seminar, or other meeting (when at least one author is a NASA employee)

TM and journal article/book or chapter/book
NASA-numbered report version of journal article or book chapter preprint (when at least one author is a NASA employee)

CR (Contractor Report)
Report of findings obtained from NASA-sponsored research performed by contractors or grant employees

CR and Presentation
NASA-numbered report version of findings presented orally at a conference, symposium, seminar, or other meeting (when the author is a NASA contractor or grantee)

CR and journal article/book or chapter/book
NASA-numbered report version of journal article or book chapter preprint (when the author is a NASA contractor or grantee)

TP (Technical Publication)
Completed research or significant phase of research, usually including extensive data or theoretical analysis (The TP is equivalent to a peer-reviewed formal professional paper, but with no length restriction.)

CP (Conference Publication)
Collection of papers from NASA-sponsored scientific and technical conferences, symposia, seminars, or other meetings sponsored or cosponsored by NASA (A CP is always a compilation, not an individual paper.)

SP (Special Publication)
Scientific, technical, management, or historical information from NASA programs, projects, or missions, often having substantial general public interest

Not NASA published, no NASA report number
Classifications of reports not published by NASA or given a NASA report number, but entered into the NASA data base, announced, and archived

Presentation
Material presented orally at a conference, symposium, seminar, or other meeting, not published by NASA or given a NASA report number (This kind of report is entered into the NASA data base, announced, and archived; the author is recognized ("credited") as having produced the report.)

Identifying information for presentations and for articles submitted to journals
Meeting title, location, date, sponsor, paper number, journal title, publisher

Preprint journal article/book or chapter/book
Material submitted to a journal or book publisher, not published by NASA or given a NASA report number (This kind of report is entered into the NASA data base, announced, and archived; the author is recognized ("credited") as having produced the report.)

4. Distribution
Nonstandard distribution (normal)
Distribution to NASA libraries, Center for AeroSpace Information ( CASI), National Technical Information Service (NTIS), and Government Printing Office

Standard distribution
Distribution to domestic and international subscribers, NASA libraries, Center for AeroSpace Information (CASI), National Technical Information Service (NTIS), and Government Printing Office (requires editing and an Export Control Information System (ECIS) number for export)

Export Control Information System (ECIS) number
Tracking number to control the distribution of information and protect sensitive technologies from unauthorized disclosure. Because Standard distribution reports are distributed outside the U.S., they are considered exports.

Restricted distribution
Distribution to qualified recipients based on the ITAR, EAR, or confidential commercial availability category marked in block III of NF1676

Local distribution
Copies for the author to distribute in addition to Nonstandard, Standard, or Restricted distribution

Credit only
For reports not published by NASA, credit given to the author. There is no distribution. Becomes part of NASA's data base, and is announced and archived in NASA CASI

5. Do you want this report edited?
Mark a box to indicate whether or not you want this report to be edited. Standard distribution reports must be edited unless division chief waives editing. A range of editing services is available in Publishing Services (Logistics and Technical Information Division), from low-level edits for spelling and grammar to high-level edits for clarity and organization.

Contract or grant number under which research was funded when work was performed by a contractor or grantee (e.g., NAS-3 12345 or NAG3-1234)

Monitor
Glenn civil service employee who supervised the contract under which this STI was generated

Monitor's phone number
Onsite number at which the contract monitor can be reached during business hours

Monitor's mail stop
Mail stop where material and final reports will be mailed

Monitor's org. code
The contract monitor's assigned organization code

Monitor's E-mail address
Electronic address to which information about the report may be sent
